What is all about PGD Genetic Testing?

PGD stands for preimplantation genetic testing/ Diagnosis, it comprises a group of genetic assays used to evaluate the embryo before transferring it to the uterus. Preimplantation genetic testing is a highly modified technique which is used to identify genetic defects in embryo created through the IVF in vitro fertilization before pregnancy, preimplantation genetic diagnosis refers specially when one or both genetic parents has any known genetic abnormality and the testing is generally performed on an embryo to determine if the embryo also carries any genetic abnormality. In this procedure the embryos can be tested for abnormal chromosomes before they are transferred to uterus, Along with this preimplantation genetic screening also referred as PGS is a technique where the embryo from the presumed chromosomally normal genetic parents are screen for aneuploidy.

Because only the uninfected embryos are transferred to the uterus for implantation preimplantation genetic testing provides alternative to current post conception diagnostic procedure that is chronic villus sampling or amniocentesis. Preimplantation genetic testing aur diagnosis is the only option available that is used to avoid high risk of having children with any genetic disorder prior to implantation, PGD is an attractive means of preventing heritable genetic diseases thereby it helps to eliminate the dilemma of pregnancy termination following unfavourable prenatal diagnosis.

Common diseases diagnosed through PGD:-

  • BRAC1 BRAC 2 genetic mutation
  • Hemophilia A
  • Sickle cell Anemia
  • Myotonic dystrophy
  • Fragile x syndrome
  • Duchenne muscular dystrophy (DMD)
  • Cystic fibrosis (CF)
  • Human immunodeficiency virus(HIV)
  • Tay Sachs disease

Indications of PGD

  • Advanced maternal age (>35 years)
  • Unexplained fertility
  • Male factors
  • Known genetic causes
  • Recurrent miscarriage
  • Repeated IVF

Classification of PGD Genetic Testing:-

Basically there are three types of preimplantation genetic testing depend on defects evaluated-

1) PGT-A - this is also called as preimplantation genetic screening it helps to increase pregnancy rates by allowing the discard of aneuploids and the selection of good euploid embryos for transfer, euploid embryos are more prone to implant and develop into healthy pregnancy.

2) PGT-M - in this monogenic diseases can be evaluated in the embryo, monogenic disorders are basically caused by single gene mutation.

3) PGT-SR - average structural abnormality into the chromosome is taken inside this (translocation, inversions, duplications insertions, deletions)

How is PGD performed:-

In This procedure one or more than one cells are taken from the embryo and send for genetic testing while the embryo is already growing in the IVF laboratory, in this there are different stages of testing which is done using different techniques these techniques include fluorescent in situ hybridization (FISH), microarray and single nucleotide polymorphism (SNP). More the techniques are constantly evolving and are changing to work more accurately and it will be easier to access.

Benefits of Preimplantation genetic testing:-

As Preimplantation genetic testing is done in the chromosomes it can help to identify any diseases linked to chromosome genetics or hereditary diseases, chromosome diseases are one of the most common and major cause of infertility and maternal age related reduced fertility potential, preimplantation genetic testing can help to shorten the time to a viable pregnancy by reducing the need of multiple in vitro fertilization cycles. It can also help to reduce the miscarriage risk independent of maternal age.

  • Helps to identify and avoid fertilisation embryos that carry genetic diseases and conditions
  • Identify the sex of the embryo and select which sex to implant.
  • Helps to allow intended parents who are carriers of genetic disorders to conceive with significantly lower risk of passing that genetic disorders to the children
  • Helps to improve the chances of successful pregnancy by transferring strongest and the best embryos in the uterus.

Risks of treatment:-

Generally the risk involved in in implantation genetic technique are similar to conventional in vitro fertilization IVF-

  • Fertility drug reaction which includes hot flashes headaches restlessness feeling down or irritable the symptoms of this usually disappears after a short time.
  • Multiple pregnancy is a risk of complication in which there are higher chances of twins or multiples
  • Pelvic infections
  • Ectopic pregnancy ab pregnancy is another risk factor in which the pregnancy development is in the fallopian tube instead of the womb
  • Miscarriage there are risk of miscarriage after IVF is no higher or lower then after a neutral conception.

Does fertility decrease after the age of 35?

Fertility generally starts to reduce after a woman is in her late 30’s or more after the age of 35 therefore the chances of getting pregnant in any monthly cycle after 35 is nearly about 5% that is very low. Chances of fertility drop down up to 10% in the ’30s and later that number goes down up to 5% per month in her 40’s. so eventually result in a decrease in fertility but always begin well till menopause. Pregnancy in the late ’30s is termed as ‘Geriatric pregnancy” a rarely used term. The reason for fertility decline after the age of 35 is there are main two factors there is a decrease in egg count as women lose eggs each month.

Some women face problem with running out of eggs and also the remaining eggs in older women are more likely to have abnormal chromosomes (mistakes in their DNA) and the aged women is at higher risk of disorders such as uterine fibroids, endometriosis, etc. some signs of poor egg quality are high FSH level, low AMH level, low antral follicle count & history of poor response during stimulation. With poor egg quality, fertility decreases & could have difficulty conceiving. If she has a large quantity but poor-quality eggs but having a good supply of eggs can be helpful because having a large quantity usually implies that some eggs will be viable in women. And at later 35 the body started to prepare itself for menopause.

Past 35 fertility started declining faster, difficulty to get pregnant naturally and most women rely on ARTs, with the beginning of perimenopause most women rely on IVF per cycle 1-5%, per year 40%, miscarriage 34-53 %. Higher the age higher the risk of complications. These facts vary from person to person for such conditions basic safety precautions can help maintain a healthy diet & weight, restrain yourself from alcohol and smoking consumption, limit caffeine. Make a pre-conceptional check-up, track ovulation, take fertility supplements in professional guidance. These are some basic safety precautions and guidance one can follow to improve remaining fertility.

To know more about your condition one needs professional guidance, consult an infertility specialist very quickly. Before its late visit 24 hours available, best in the city specialist.

REASON OF REPETITIVE FAILURE OF IVF AND WHAT TO DO NEXT?

Failure of IVF is very disheartening, but it is not the end. Still you have the hope to get successful IVF. We know a lot of people who struggle to conceive and opt for In Vitro Fertilization (IVF) treatment. With proper guidance of your IVF specialist, most couples get the gift of parenthood, it is a joyful experience and the hurdles doesn’t matter when the couple achieve it.

Here we will discuss the different causes of IVF failures and how to approach for the next step. But the process IVF is not the only thing that matters, we have encountered with a lot of couples who have faced repeated IVF failures.

Common Causes Of Repeated IVF Failures

  • Poor Endometrium status: – Thin endometrium lining is not suitable for embryo implantation.

  • Uterine Abnormality: – Uterine fibroid/cysts/endometrial polyps/septum and so on, can be the cause of implantation failures.

  • Embryo Quality: – Poor quality of embryo or chromosomal abnormality in embryo causes difficulty in implantation, or in case if they gets implanted, they may not remain for a long time.

When embryonic layer surrounding the oocyte, known as the zona is thicker than usual then it can also leads to implantation failure. It happens because of the failure of sperms ability to break the zona during fertilization.

  • Genetic Abnormality :– Any minor defect like chromosomal microdeletions, can compromise the quality of gametes and hence embryo.

  • Maternal health: – Uncontrolled diabetes, thyroid disorders, etc., can be the conditions that may cause difficulty in pregnancy.

  • Difficulty In Embryo Transfer :– It is one of the most common reason for failed IVF procedure. Issues at embryo transfer or blood seen at catheter tips causes high risk of failure.

  • Autoimmune Diseases :– If the couple face some autoimmune diseases, there may be risk of recurrent IVF failures. Usually, screening of the couple with repeated pregnancy loss is done to rule out the chance of autoimmune disease.
How Can we Prevent Repeated IVF Failures?

Here are some tips / advice for you to prevent future IVF failures:-
  • The firstly and most importantly you need to get done a thorough examination involving ultrasound and blood tests and then screening for determination of the ovarian reserve and semen analysis.
  • After that, you need to go for a laparoscopic hysteroscopy test to look for any physical obstacles in conception.
  • Your IVF specialist will freshen the poor-quality endometrium; also, you have to send a sample to rule out the chance of tuberculosis.
  • Hydrosalpinges should be clipped during this time to avoid the embryo-toxic fluid from causing IVF failures.
  • After following these steps, your IVF specialist will help you and start the right ovarian stimulation.
  • Your IVF specialist will make sure that your endometrium is fit for implantation in quality and thickness.
Even after this also you have continuous failure of IVF then also not to be worried. There are various treatments like ICSI and IUI that can help you to boost your IVF treatment procedure.

Make sure that you choose the best IVF centre for getting your IVF treatment done and from an experienced IVF specialist. The experience of an IVF clinic affects the results of the treatment.

Causes of fibroids-

The uterine fibroid is an unpredictable condition that gets severe over the time if left untreated  in women's. Here are some of the most common causes of uterine fibroids-

  • Hormones - The main two hormones that are responsible to  stimulate the development of uterine lining during pregnancy and menstrual cycles are oestrogen and progesterone, are more likely to appear in the promotion of overgrown fibroids. It is often observed that after the menopause the fibroids tend to shrink due to lack of production of these hormones.

  • Genetic changes - Many of the fibroids contains a sudden change in genes that is often observed in some of the womens that is different from normal uterine muscle cells these changes in genetics can cause fibroids.

  • Family history - The fibroids may run in the family. If in the family there is a history e off fibrosus  to your mother sister and grandmother there are high chances you also may develop it as well.

Medications include-
  • Gonadotropin- releasing hormone(GnRH) agonists:- these helps in blocking the production of estrogen and progesterone, and put them in temporary menopause like stage.
GnRH include leuprolite(lupron Depot, eligard, others), goserelin, and triptorelin.
  • Progestin- releasing intrauterine device(IUD):- these help in relieving extra bleeding that is caused by fibroids.
  • Tranexamic acid:- these are the non hormonal medication that is taken to ease the heavy menstrual periods.
2) Fibroid surgery:-

Myomectomy is the procedure that helps to remove the fibroids without uterus damage there are several types of myomectomy, which procedure will be the best depends on where the fibroids are present, the type of myomectomy procedure that helps in removal of fibroids includes -
  • Hysteroscopy - this is the procedure done by inserting a scope in the vagina and the cervix into the uterus this is used to cut away the fibroids using the scope.

  • Blood tests - Common blood test will be done to analyze and to measure hormone levels. This testing will help to exclude possible causes of menstrual abnormalities or androgen exces. There will be an additional blood testing to measure glucose tolerance and fasting cholesterol and triglyceride levels.
  • An ultrasound - The doctor first checks the appearance of the ovaries and also checks the thickness of the lining of the uterus.
