Intracytoplasmic sperm injection

 What Is ICSI?

ICSI is an assisted reproductive technology that involves the injection of a single live sperm into an egg. The technique is generally recommended for cases of male factor infertility, or for couples who have had recurrent failed in vitro fertilisation (IVF) cycles. The technology is used to overcome several fertility issues.

Intracytoplasmic Sperm Injection (ICSI) is a specialised form of In Vitro Fertilisation (IVF) that is used primarily for the treatment of severe cases of male-factor infertility. ICSI involves the injection of a single sperm directly into a mature egg.

Before a man’s sperm can fertilize a woman’s egg, the head of the sperm must attach to the outside of the egg. Once attached, the sperm pushes through the outer layer to the inside of the egg (cytoplasm), where fertilization takes place.

Sometimes the sperm cannot penetrate the outer layer, for a variety of reasons. The egg’s outer layer may be thick or hard to penetrate or the sperm may be unable to swim. In these cases, a procedure called Intracytoplasmic sperm injection (ICSI) can be done along with in vitro fertilization (IVF) to help fertilize the egg. During ICSI, a single sperm is injected directly into the cytoplasm the egg.

ICSI helps to overcome fertility problems, such as:
  • The male partner produces too few sperm to do artificial insemination (intrauterine insemination [IUI]) or IVF.

Infertility treatment :-

 Not getting pregnant, despite having carefully timed, unprotected sex for one year.

The cause of infertility may be difficult to determine but may include inadequate levels of certain hormones in both men and women, and trouble with ovulation in women.

Many treatments significantly improve the chances of getting pregnant. They include hormone treatments, fertility drugs and surgery. In addition, assisted reproduction uses various medical techniques to fertilise an egg.

Men, fertility is treated with:

  • Add caption
    Surgery, if the cause is a varicocele (widening of the veins in the scrotum) or a blockage in the vasa deferens, tubes that carry sperm.
  • Antibiotics to treat infections in the reproductive organs.
  • Medications and exercise to treat problems with erections .
  • Hormone treatments if the problem is a low or high level of certain hormones.

Women, infertility is treated with:
  • Fertility drugs and hormones to help the woman ovulate or restore levels of hormones
  • Surgery to remove tissue that is blocking fertility (such as endometriosis) or to open blocked fallopian tubes

Other more treatment for infertility in men and women both :-

Infertility in men and woman can also be treated with Assisted Reproductive Technology, or ART. There are several types of ART:

IUI (intrauterine insemination): Sperm is collected and the placed directly inside the woman’s uterus while she is ovulating.

IVF (in vitro fertilization): The sperm and egg are collected and brought together in a lab. The fertilized egg grows for 3 to 5 days . Then the embryo is placed in the woman’s uterus.

GIFT (gamete intrafallopian transfer) and ZIFT (zygote intrafallopian transfer): The sperm and egg are collected and quickly placed in a fallopian tube. With GIFT, the both sperm and eggs are placed into the fallopian tube. With ZIFT, the sperm and eggs are brought together in a lab and then a fertilized egg is placed into the tube at 24 hours.

Gonadotropin injection :-

Once the follicles (fluid filled sacs containing the eggs) reach a mature size, an HCG injection is administered which leads to final development and maturation of the eggs. Just before those eggs would otherwise be ovulated, they are retrieved under mild anaesthesia in an operating room. This procedure is done by ultrasound guidance when the surgeon utilizes a narrow needle to retrieve eggs from the ovary through the vaginal wall. This sterile needle is attached to sterile suction tubing and a collecting vial. Once the fluid containing the eggs is removed from the follicles into the vials, they are handed to the embryologist who finds the eggs, places them in tiny droplets on a Petri dish, and then fertilizes the eggs using their partner’s or donor sperm. The sperm can either be mixed with the eggs to allow normal fertilization (conventional insemination) or by injecting one sperm into each mature egg (ICSI).
