Polycystic ovary syndrome (PCOS)

 A hormonal disorder causing enlarged ovaries with small cysts on the outer edges.

The cause of polycystic ovary syndrome isn’t well understood, but may involve a combination of genetic and environmental factors. Polycystic ovary syndrome (PCOS) is a hormonal disorder common among women of reproductive age. Women with PCOS may have infrequent or prolonged menstrual periods or excess male hormone (androgen) levels. The ovaries may develop numerous small collections of fluid (follicles) and fail to regularly release eggs.

Way to examine polycystic ovary syndrome :-

A pelvic exam. The doctor may visually and manually inspects your reproductive organs for masses, growths or other abnormalities.

Blood tests. Your blood may be analysed to measure hormone levels. This testing can exclude possible causes of menstrual abnormalities or androgen excess that mimics PCOS. You might have additional blood testing to measure glucose tolerance and fasting cholesterol and triglyceride levels.

An ultrasound. Your doctor checks the appearance of your ovaries and the thickness of the lining of your uterus. A wandlike device transducer is placed in your vagina (transvaginal ultrasound). The transducer emits sound waves that are translated into images on a computer screen.

  • Menstrual disorders: PCOS mostly produces oligomenorrhea (fewer than nine menstrual periods in a year) or amenorrhea (no menstrual periods for three or more consecutive months), but other types of menstrual disorders may also occur.
  • Infertility: This generally results directly from chronic anovulation (lack of ovulation).
  • High levels of masculinizing hormones: Known as hyperandrogenism, the most common signs are acne and hirsutism (male pattern of hair growth, such as on the chin or chest), but it may produce hypermenorrhea (heavy and prolonged menstrual periods), androgenic alopecia (increased hair thinning or diffuse hair loss), or other symptoms. Approximately three-quarters of women with PCOS have evidence of hyperandrogenemia.
  • Metabolic syndrome: This appears as a tendency towards central obesity and other symptoms associated with insulin resistance. Serum insulin, insulin resistance, and homocysteine levels are higher in women with PCOS.

In vitro fertilization

In vitro fertilisation is a process of fertilisation where an egg is combined with sperm outside the body, in vitro. The process involves monitoring and stimulating a woman’s ovulatory process, removing an ovum or ova from the woman’s ovaries and letting sperm fertilise them in a liquid in a laboratory.

The first step in IVF is taking fertility medications for several months to help your ovaries produce several eggs that are mature and ready for fertilization. This is called ovulation induction. You may get regular ultrasounds or blood tests to measure your hormone levels and keep track of your egg production.

Once your ovaries have produced enough mature eggs, your doctor removes the eggs from your body (this is called egg retrieval). Egg retrieval is a minor surgical procedure that’s done at your doctor’s office or at a fertility clinic.

  • Unexplained infertility. Unexplained infertility means no cause of infertility has been found despite evaluation for common causes.
  • A genetic disorder. If you or your partner is at risk of passing on a genetic disorder to your child, you may be candidates for preimplantation genetic testing — a procedure that involves IVF. After the eggs are harvested and fertilized, they’re screened for certain genetic problems, although not all genetic problems can be found. Embryos that don’t contain identified problems can be transferred to the uterus.
  • Fertility preservation for cancer or other health conditions. If you’re about to start cancer treatment — such as radiation or chemotherapy — that could harm your fertility, IVF for fertility preservation may be an option. Women can have eggs harvested from their ovaries and frozen in an unfertilized state for later use. Or the eggs can be fertilized and frozen as embryos for future use.
  • Women who don’t have a functional uterus or for whom pregnancy poses a serious health risk might choose IVF using another person to carry the pregnancy (gestational carrier). In this case, the woman’s eggs are fertilized with sperm, but the resulting embryos are placed in the gestational carrier’s uterus.

Intracytoplasmic sperm injection

 What Is ICSI?

ICSI is an assisted reproductive technology that involves the injection of a single live sperm into an egg. The technique is generally recommended for cases of male factor infertility, or for couples who have had recurrent failed in vitro fertilisation (IVF) cycles. The technology is used to overcome several fertility issues.

Intracytoplasmic Sperm Injection (ICSI) is a specialised form of In Vitro Fertilisation (IVF) that is used primarily for the treatment of severe cases of male-factor infertility. ICSI involves the injection of a single sperm directly into a mature egg.

Before a man’s sperm can fertilize a woman’s egg, the head of the sperm must attach to the outside of the egg. Once attached, the sperm pushes through the outer layer to the inside of the egg (cytoplasm), where fertilization takes place.

Sometimes the sperm cannot penetrate the outer layer, for a variety of reasons. The egg’s outer layer may be thick or hard to penetrate or the sperm may be unable to swim. In these cases, a procedure called Intracytoplasmic sperm injection (ICSI) can be done along with in vitro fertilization (IVF) to help fertilize the egg. During ICSI, a single sperm is injected directly into the cytoplasm the egg.

Infertility treatment :-

 Not getting pregnant, despite having carefully timed, unprotected sex for one year.

The cause of infertility may be difficult to determine but may include inadequate levels of certain hormones in both men and women, and trouble with ovulation in women.

Many treatments significantly improve the chances of getting pregnant. They include hormone treatments, fertility drugs and surgery. In addition, assisted reproduction uses various medical techniques to fertilise an egg.

With GIFT, the both sperm and eggs are placed into the fallopian tube. With ZIFT, the sperm and eggs are brought together in a lab and then a fertilized egg is placed into the tube at 24 hours.

Gonadotropin injection :-

Once the follicles (fluid filled sacs containing the eggs) reach a mature size, an HCG injection is administered which leads to final development and maturation of the eggs. Just before those eggs would otherwise be ovulated, they are retrieved under mild anaesthesia in an operating room. This procedure is done by ultrasound guidance when the surgeon utilizes a narrow needle to retrieve eggs from the ovary through the vaginal wall. This sterile needle is attached to sterile suction tubing and a collecting vial. Once the fluid containing the eggs is removed from the follicles into the vials, they are handed to the embryologist who finds the eggs, places them in tiny droplets on a Petri dish, and then fertilizes the eggs using their partner’s or donor sperm. The sperm can either be mixed with the eggs to allow normal fertilization (conventional insemination) or by injecting one sperm into each mature egg (ICSI).

What is polycystic ovary syndrome?

What is PCOS?

Most women with PCOS grow a number of small cysts, or fluid-filled sacs, on their ovaries. The cysts are not harmful, but they can lead to an imbalance in hormone levels.

Women with PCOS may also experience menstrual cycle abnormalities, increased androgen (sex hormone) levels, excess hair growth, acne, and obesity.

In addition to the many health conditions associated with PCOS, which will be discussed in this article, PCOS is the most common cause of infertility in women – because it can prevent ovulation.

Women who can conceive with PCOS have a higher incidence of miscarriage, gestational diabetes, pregnancy-induced high blood pressure, preeclampsia, and premature delivery.

Causes

Currently, there is no known cause of PCOS. However, there are associations with excess insulin, low-grade inflammation, and genetics.

Treatment

There is no cure for PCOS, but treatment aims to manage the symptoms that affect an individual.

This will depend on whether the individual wants to become pregnant and aims to reduce the risk of secondary medical conditions, such as heart disease and diabetes.

There are several recommended treatment options, including:

Birth control pills: These can help regulate hormones and menstruation.

Diabetes medications: These help manage diabetes, if necessary.

Fertility medications: If pregnancy is desired, these include the use of clomiphene (Clomid), a combination of clomiphene and metformin, or injectable gonadotropins, such as follicle-stimulating hormone (FSH) and luteinizing hormone (LH) medications. In certain situations, letrozole (Femara) may be recommended.

Fertility treatments: These include in-vitro fertilization (IVF) or inseminations.

Excessive hair growth may be reduced with the drug spironolactone (Aldactone) or eflornithine (Vaniqa). Finasteride (Propecia) may also be recommended, but it should not be handled by women who may become pregnant.

Anyone using spironolactone should use birth control, due to the risk of birth defects if taken while pregnant. Breast-feeding on this medication is not recommended.

Other possible options to manage hair growth is laser hair removal, electrolysis, hormonal treatments, or vitamin and mineral use.

Surgical options include:

Ovarian drilling: Tiny holes made in the ovaries can reduce the levels of androgens being produced.
Oophorectomy: Surgery removes one or both ovaries.
Hysterectomy: This involves removal of all or part of the uterus.
Cyst aspiration: Fluid is removed from the cyst.

Pregnancy and menopause

Women who are affected by PCOS may experience the effects throughout their lifetime.

There may be an increased risk of miscarriage, gestational diabetes, preeclampsia, and preterm births.

After delivery, there is an increased risk of the newborn being placed in the neonatal intensive care unit or death before, during, or soon after birth. These complications are more common in multiple births, for example, twins or triplets.

Symptoms such as excessive hair growth and male pattern baldness can last beyond menopause and may become worse.

With aging also comes the risk of the secondary health complications related to PCOS, including heart disease.

Types of embryo transfer

In order to extract and fertilize the eggs during IVF, doctors generally follow the same process every
time. Once fertilization has occurred, there are a few different options available for embryo transfer:

Fresh embryo transfer: Once eggs have been fertilized, they are cultured for 1-2 days. The best embryos are chosen to transfer directly to the woman’s uterus.

Frozen embryo transfer: Any healthy embryos that were not used in the first transfer can be frozen and stored for future use. These can be thawed and transferred to the uterus.

Blastocyst embryo transfer: If many healthy embryos develop after the fertilization, it is common to wait to see if the embryos develop into blastocysts. According to a study in the Indian Journal of Clinical Practice, blastocyst embryo transfer has a higher success rate than the standard embryo transfer on day 3. However, another recent study suggests that it may pose risks later in pregnancy and should not always be recommended.

Assisted hatching (AH): A study in the Reproductive Biomedicine Online found that the process of assisted hatching – weakening the outer layer of the embryo before it is transferred to the uterus – does not improve pregnancy and implantation rates in women who are having fresh embryos transferred. The researchers noted, however, that women having frozen embryos implanted do benefit from having their embryos treated in this way.
