Pretreatment with dienogest in women with endometriosis undergoing IVF after a previous failed cycle

 Highlights

  • IVF has worse outcomes in patients with endometriosis.

Abstract

Research question

The major causes of IVF failure in women with endometriosis have been attributed to decreased ovarian reserve, low embryo quality and impaired receptivity of the endometrium. Dienogest (DNG) has anti-inflammatory and anti-angiogenic activity and so may theoretically improve IVF outcomes in women with endometriosis. This study aimed to evaluate the administration of DNG before IVF in women with endometriosis who had previously failed one IVF cycle.

Methods

This study was based on the retrospective analysis of a prospectively collected database, including 151 women who had failed a previous IVF cycle and all subsequent embryo transfers and had an imaging diagnosis of endometriosis. Patients either directly underwent IVF without receiving hormonal treatment or received 3 months of treatment with DNG (2 mg/daily) before IVF.

Results

Eighty-eight (58.3%) patients underwent IVF without previous hormonal treatment, and 63 (41.7%) received pretreatment with DNG. The cumulative implantation, clinical pregnancy and live birth rates were significantly higher in the DNG-treated group (39.7%, 33.3% and 28.6%) than in the non-treated group (23.9%, 18.2% and 14.8%; P = 0.049, 0.037 and 0.043, respectively). The largest diameter of endometriomas significantly decreased after DNG pretreatment (P < 0.001). The use of DNG increased significantly the number of oocytes retrieved (P = 0.031), two-pronuclear embryos (P = 0.039) and blastocysts (P = 0.005) in women with endometriomas of diameter ≥4 cm.

Conclusions

This study suggest that in patients with endometriosis, IVF outcomes can be improved by pretreatment with DNG. In particular, the use of DNG allows for better oocyte retrieval and blastocysts in patients with large endometriomas.

Infertility and It’s Causes in women

Infertility means not being able to become pregnant after a year of trying. If a woman can get pregnant but keeps having miscarriages or stillbirths, that’s also called infertility. Infertility is fairly common. After one year of having unprotected sex, about 15 percent of couples are unable to get pregnant.

Causes of infertility in women

There are many reasons of infertility in women some of them are: -

  • Blocked fallopian tube :- Blocked fallopian tubes due to pelvic inflammatory disease, endometriosis, or surgery for an ectopic pregnancy.
  • Problem in uterus :- Physical problems with the uterus.Uterine fibroids, which are non-cancerous clumps of tissue and muscle on the walls of the uterus.
Damage to your fallopian tubes. These structures carry eggs from your ovaries, which produce eggs, to the uterus, where the baby develops. They can get damaged when scars form after pelvic infections, endometriosis, and pelvic surgery. That can prevent sperm from reaching an egg in the tube. The egg and sperm meet in the tube. This is where the egg is fertilized and then moves down to the uterus to implant.
  • Hormone problem :- Hormonal problems. You may not be getting pregnant because your body isn’t going through the usual hormone changes that lead to the release of an egg from the ovary and the thickening of the lining of the uterus. Hormonal problems can sometimes cause or foster infertility, and the levels of several key hormones may be tested during an infertility evaluation. Of these, progesterone is the most critical and usually the first hormone tested. Progesterone levels generally peak one week after ovulation, and can be measured through blood tests to detect the amount of progesterone present in the blood stream.
Hormone in females body

Follicle stimulating hormone (FSH) helps control the menstrual cycle and egg production. Estradiol increases during the menstrual cycle and is a marker of ovarian function and quality of eggs. These hormones are evaluated early in the menstrual cycle to evaluate ovarian reserve, along with anti- mullerian hormone and an ultrasound of the ovaries. Estradiol is also frequently evaluated during treatment cycles to assess growth and quality of the developing egg.

Luteinizing hormone (LH) is another critical hormone that is frequently tested, usually in a urine sample just before ovulation occurs. LH stimulates the ovaries to release an egg so urinary measurement of this hormone is helpful to time intercourse or intrauterine insemination.
  • Cervical issue:- Cervical issues. Some women have a condition that prevents sperm from passing through the cervical canal.
  • Uterine trouble:- Uterine trouble. You may have polyps and fibroids that interfere with getting pregnant. Uterine polyps occur when too many cells grow in the endometrium, the lining of the uterus. Fibroids grow in the wall of the uterus.

In-vitro maturation versus IVF: a cost-effectiveness analysis

 Abstract

Research question

How do costs and effects of in-vitro maturation (IVM) compare to IVF in women with a high antral follicle count (AFC)?

Design

This cost-effectiveness analysis (CEA) was based on data of a previous retrospective cohort study at IVFMD, My Duc Hospital, Ho Chi Minh City, Vietnam. Between July 2015 and December 2017, 608 women underwent IVM and 311 women IVF. The effectiveness measure for the CEA was cumulative live birth rate (LBR) after one completed cycle including subsequent cryo-cycles within 12 months of inclusion. Data were collected on resource use related to treatment, medication and pregnancy from the case report forms. The mean costs and effects, average cost differences and incremental cost-effectiveness ratios (ICER) were calculated using non-parametric bootstrap resampling to assess the effect of uncertainty in the estimates.

Results

Cumulative LBR after one completed cycle were 239/608 (39.3%) in the IVM group versus 155/311 (49.8%) in the IVF group (adjusted odds ratio 0.52, 95% confidence interval [CI] 0.30–0.89). Ovarian hyperstimulation syndrome (OHSS) did not occur in the IVM group versus 11/311 (3.5%) in the IVF group. The mean costs per couple were €4300 (95% CI €1371–18,798) for IVM and €6493 (95% CI €2204–20,136) for IVF. The ICER per additional live birth with IVF was €20,144 (95% CI €9116–50,418). Results were robust over a wide range of assumptions.

Conclusions

IVM is less expensive than IVF in women with a high AFC undergoing treatment with assisted reproductive technology, while leading to a slightly lower effectiveness in terms of cumulative LBR.
